This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.
Under the general supervision of the Manager of Continuing Care Programs, the Continuing Care Coordinator is responsible for the intake, assessment, and identification of unmet needs. Duties include carrying out home visits as well as participation in program education, promotion and development activities. Responsibilities also include care/service planning and authorization, ongoing case management, evaluation and appropriate discharge planning of clients. The Continuing Care Coordinator works in community, hospital and long term care settings in partnership with health care professionals, designated service providers, government departments and other community agencies and resources.
Bachelor’s Degree in Nursing. Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies plus 5 years related experience. (A diploma prepared nurse (RN) with 9 years experience in the process of completing a degree will be considered.)
Two years of experience working within a community setting
Related experience consists of working in the community with geriatrics as a focus.
Registration (or eligibility) with applicable professional association is required.
Demonstrated ability in assessing, interviewing and negotiating is required.
Demonstrated ability to make sound decisions regarding client care and to work cooperatively and effectively with other professionals is required.
Demonstrated ability in oral and written communication is required.
Demonstrated ability in computer skills is essential.
Demonstrated ability to multi-task and prioritize workload is essential.
Must have excellent assessment, interviewing and negotiation skills which are essential in dealing with complex client situations and care plans.
Must be willing to work in the hospital as well as the community.
Must possess a good knowledge of community agencies.
Must be adaptable, flexible and willing to take on new assignments.
Must have a valid driver’s license and access to a reliable vehicle is required.
Previous work attendance will be considered
Competencies in other languages an asset, French preferred
Hours of Work
Permanent Part-time Position; 35 hours bi-weekly
$38.4501 – $43.7766 hourly
